Benefits Description
Health
Insurance
PLC
offers Medical, Dental, and Vision Insurance to all eligible employees and
their eligible dependents.
At the time of eligibility, each employee may choose a plan in
which he/she want to be enrolled.
All full-time employees are eligible for enrollment the first day
of the month following 30 days of continuous employment.
Flexible
Spending Account
Employees
may elect to have money deducted from their paychecks on a pretax basis to
pay for dependent care and uncovered medical expenses.
This program is currently administered by Ceridian.
All full-time employees will be eligible to enroll in the Dependent
Care Account immediately following date of employment and will be eligible
for enrollment in the Health Care Account on the first day of the month
after 30 days of continuous employment.
Life
and AD&D Insurance
All
full-time employees are eligible to enroll immediately following date of
employment. PLC
contributes 100%.
Short
& Long Term Disability
All
full-time employees are eligible to enroll immediately following date of
employment. PLC
contributes 100%.
Pension
Plan 403(B)
This
is a qualified retirement plan provided by ING in which PLC contributes 5%
of gross salary for full-time employees.
Employees are eligible for enrollment upon one full year of
employment with PLC.
All
employees can immediately enroll into a tax deferred supplemental
retirement annuity through ING.
This is not an employer contributory plan.
General
Leave
PLC
provides paid leave for full-time employees based upon length of service.
General Leave increases as follows:
During the
first three years of employment, employees earn 20 days per year.
4-8 years of
employment, 25 days per year.
9 years and
each year thereafter, 30 days per year.
Part-time
employees who work a minimum of 20 hours on a regular scheduled per week
will receive prorated leave.
Holidays
PLC
grants all full-time employees ten (10) paid holidays.
Jury/Witness
Duty
An
employee required to report for jury duty or to appear under a subpoena as
a witness on a scheduled workday will receive normal compensation provided
official documentation is presented.
If hours of work do not conflict with jury service, the employee is
expected to report to work for regular work hours.
Bereavement
Leave
PLC
will allow paid bereavement leave for up to three working days in order to
allow employees to attend services if there is a death in an employee’s
immediate family.
Statutory Benefits
Progressive
Life Center provides all statutory benefits and/or required programs
according to federal, state, and local laws.
These benefits vary between states and local jurisdictions.
Workers
Compensation
This
insurance provides medical coverage and partial salary continuation should
you become temporarily or permanently disabled from doing your job due to
an injury or condition arising from your employment.
Unemployment
Insurance Benefits
This
is a state required benefit that ensures a salary continuance for eligible
workers who become unemployed due to no fault of their own.
Eligibility for this benefit, and the level of benefits provided
vary between states.
Social
Security
This
is a federal program that provides a variety of benefits to you and/or
your beneficiaries when you retire or if you become permanently disabled
or die. PLC
and the employee make contributions to this statutory program.
