Progressive
Life Center (PLC) has been providing Family Preservation Services in
Montgomery County since June 2005. The ultimate goal of the program is to minimize the risk
of children being removed from their family and home. Intensive,
in-home services are provided to build upon existing family strengths,
minimize family crisis, stabilize the family unit, and keep the family
in tact. Counselors work up close and personal with families to
identify strengths and needs, and develop plans of care specifically
tailored for the family. Families are involved in all aspects of
treatment and intervention. Family preservation incorporates the
Homebuilders Service Delivery Model with NTU Service Delivery Model.

- Crisis Prevention/Intervention
- Case Management
- Individual and Family Counseling
- Community Linkages
- Parent Training and Education
- Step Down Services
- Discharge Planning

- Quality and Accurate Documentation
- On-Site Food Pantry

4-12 weeks.
- Services are provided in home.
- Services are voluntary.
- Services are 24-7 (around the clock).
- Services are provided during flexible hours to
accommodate individual scheduling needs.
- Services are culturally sensitive and
competency based.
- Services are immediate. Contact is made with
families within 24 hours of referral.
